The ICM Director provides leadership and oversight, supporting and executing strategic initiatives with division and corporate executive teams for multiple divisions/areas of responsibilities. Directs and oversees the efficient and effective operations for multiple hospital divisions and/or system functions. Oversees the management of care transitions in partnership with the multidisciplinary team in the hospital setting and is responsible for the delivery of high quality care and outcomes across their areas of responsibility. The ICM Director leads, directs and manages high performance teams with accountability for the alignment and execution of strategic goals related to service delivery, growth, staffing, policies and procedures, employee development, performance management, customer service/satisfaction, quality and patient safety, regulatory requirements (CMS, DNV, state, etc.).
Education
- BSN (Required)
- Masters Degree in Nursing, Social Work, Business or Finance (Required)
Certification/Licensure
- Registered Nurse – VA single state or Multi/Compact State
- Must maintain a Certification in Case Management (ADM, CCM, CMAC or CMGT-BC)
Experience
- 3 years as a Supervisor, Manager or Director in acute care or case management required
- 5 yeas of case management experience required
