Home Care- Occupational Therapist
Overview
Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, improve or correct disabling conditions resulting from disease or injury and help build or restore vocational/ daily living skills as well as general independence.
Education
- Bachelors Degree in Occupational Therapy (Required)
- Masters in Occupational Therapy (Preferred)
or
- Doctorate in Occupational Therapy
Certification/Licensure
- License: Certified Occupational Therapist (Required)
Experience

Provides age-appropriate education for clients and caregivers.

Supervise and assume responsibility for care provided by identified team members.

Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.

Provides effective instruction to the patient and family through standardized educational programs and/or individualized training. Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.

Productive and efficient in daily operations.

Selects and performs the appropriate assessment procedures.

Establishes and updates formal plan of care to address individual patient needs.
For PACE and Resource Pool, incumbents must have at least one year of experience working with the frail or elderly population.
Must possess State License and NBCOT Certification.
Valid NBCOT Certified Occupational Therapist – Certifications – National Board Certification In Occupational Therapy (NBCOT) and license required in state of practice.
