Overview
The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.
Education
- Trade School Graduate (Required)
Certification/Licensure
- LPN License (Required) – Active in Virginia
- BLS (Required within 90 days)
Experience
- Serve as patient advocate in the prevention of illness or disease
- Provide care and treatment for patients and clinical support and teaching for patient and family members
- Customer service, testing, medication administration and the implementation of nursing interventions and care plans
- Support a holistic approach to patient care
