Sentara is seeking an Office-Based Nurse Practitioner or Physician Assistant to join Sentara Cardiology Specialists located in the beautiful mountains of Harrisonburg, VA. Join our dedicated Physicians and APPs as they provide a team-based collaborative approach to patient care, and are offered seamless access to the many resources available throughout the greater Sentara network for their patients.
Position Highlights:
- 100% Office Based
- Monday – Friday; 8-4:30p
- 4 hours of dedicated administrative time weekly
- General cardiology with a specialized focus on Heart Failure Management.
- Responsibilities include collaborating with post-discharge Heart Failure patients to reduce readmission rates, as well as conducting follow-up care for both Heart Failure and General Cardiology patients.
- Average of 10-12 patients daily
- Cardiology or Family Medicine experience preferred
