As a Care Coordinator you will work in your community to provide case management services to patients, families, and designated caregivers. Starting out by performing in home assessments with members you will then provide resources to assist patients and their families with personal, environmental and medical difficulties. Our mission is to meet the member’s needs, with the goal of optimizing member health care across the care continuum!
Qualifications:
– 1 year of experience in long term care (case management, care coordination)
– 4-year degree in health and human services (acceptable degrees listed below)
