Overview
The Manager of Behavioral Health Care Management provides strategic and operational leadership for the Behavioral Health Navigator team, supporting patients experiencing psychological distress across hospital settings—including the Emergency Department, Medical Floors, and Behavioral Health Units—at both Sentara and non-Sentara hospitals. This role is dedicated to ensuring that patients receive timely, appropriate behavioral health services by guiding them through a complex healthcare landscape and improving access to care across multiple hospital systems.
Key Responsibilities:

Lead, mentor, and manage the Behavioral Health Navigator team, ensuring effective coordination of care across licensed and non-licensed staff.

Oversee daily operations, workflow efficiency, quality assurance, and service excellence across multiple hospital sites.

Ensure patient progression through the continuum of care in alignment with clinical and financial outcomes.

Supervise the quality and compliance of assessments, treatment planning, and post-discharge coordination.

Monitor staff competencies, manage onboarding, develop resource planning, and implement engagement strategies.

Maintain knowledge of and ensure compliance with NCQA, DMAS, CMS, and other relevant regulatory standards.

Serve as a liaison to internal and external stakeholders, building collaborative partnerships across healthcare teams, social services, and community providers.

Partner with the Health Plan Quality team to enhance behavioral health integration into HEDIS, CAHPS, and STARS quality ratings.

Lead root cause analyses to identify performance trends and implement improvement strategies to support clinical outcomes.
Behavioral Health Navigator Oversight:
As part of this role, the Manager supports and enhances the work of Behavioral Health Navigators who serve as vital resources for patients in crisis. Their responsibilities include:

Conducting initial assessments to determine behavioral health needs and identifying appropriate treatment options.

Connecting patients with mental health providers, community resources, and support services.
-
Coordinating and scheduling follow-up appointments to ensure continuity of care.

Identifying and resolving barriers to care such as financial constraints, transportation issues, and insurance challenges.

Providing education and emotional support to patients and families, helping them understand treatment plans and available resources.
-
Collaborating with medical staff, social workers, case managers, and other healthcare professionals to ensure comprehensive care.

Advocating for patients’ mental health needs within the healthcare system and in the broader community.
Education
-
Master’s degree in one of the following: Social Work, Counseling, Clinical Mental Health Counseling, Marriage and Family Therapy, Psychology, Nursing, or a closely related field
Certification/Licensure
-
Licensed in one of the following: Licensed Clinical Social Worker, Licensed Clinical Mental Health Counselor, Licensed Marriage and Family Therapist, Licensed Professional Counselor, Licensed Psychological Associate, Psychiatric-Mental Health Nurse Practitioner in the state of Virginia
Experience
-
3 years of experience in Behavorial health, Healthcare Management, Medicaid, and Medicare

Demonstrated leadership experience in behavioral health or care management, with a strong focus on patient advocacy and systems navigation

Deep understanding of behavioral health systems, care coordination, and regulatory compliance
-
Experience with multidisciplinary collaboration and performance improvement initiatives
-
Familiarity with quality measurement systems, including HEDIS, CAHPS, and STARS, is strongly preferred

Excellent communication, problem-solving, and organizational skills
